What we need
Clarity beats guesswork
For wheelchairs: funding source, weight, seat dimensions, seat height if known, skin integrity notes, OT involvement, and intended environment.
For scooters: funding source, weight, and whether use is flat terrain or hills.
For beds: funding source, doctor’s prescription status, client height/weight, and mattress needs if pressure risk is present.
